I have now had MRI and further biopsies. The initial view is that lymph nodes are clear but SNL biopsy will still performed during surgery. There appears to be couple more small areas of cancer cells in the breast so it’s now mastectomy followed by radiotherapy, possibly chemo and hormone therapy. Not safe for surgery yet at now need consult with  plastic surgeons as plan is to reconstruct at same time. Lots to digest and get my head around but feeling a little more at ease with things.
