Does breast cancer always change how breast looks?

Hi, 

I've posted before and am waiting for an ultrasound after a mammogram as one breast is looking different to the other (on mammogram). I've had upper shoulder pain and hip pain on the right side for a long time and my head is running wild with scenarios that I have cancer that's spread. I am always tired but I have 2 small kids so put it down to that. The breast il getting seen to doesn't look very different to the other side, no dimpling or inverted nipple etc. im just wondering is it possible to have breast cancer that has spread without much changes to the breast itself? Xx
