vocal chord cancer (T1) laser treatment, need advice

Have had laser surgery in sept for early vocal chord t1 cancer and was told it was successful but a month ago I had a check up which revealed there has been a reacurrance ( though only small and on the surface) so have had more laser surgery in the hope that will do the trick as the surgeon says its too small to warrant radiotherapy. Has anyone else had this kind of treatment and is this usual?( reacurrance and more lasering). I'm very concerned that this treatment may not be working or that cancer may keep coming back, resulting in radiotherapy and the possibility of eventually loosing my voice box. I know its early days and I'm probably worrying too much but would be very grateful if there is anyone out there who can comment on this.

  • Hi Zarker

I had severe displacia of the vocal chords around 5 years ago and was told that I needed laser treatment immediately as it "was very close to cancer". I had laser treatment a few times and had to have biopsies done around every 2 - 3 months but in my case it had gone too deep so they could only laser the surface and it kept coming back. If you are a smoker you should give it up immediately as my consultant told me to avoid smoke at all costs.

I went on to have larygneal cancer but I was stage T2-3 when I was eventually diagnosed. I was successfully treated with radiotherapy 28 months ago and touch wood everything is fine and my voice is better than it's been for years.

On a positive note they are closely monitoring you and as you are at a very early stage hopefully they will be able to get rid of it with laser treatment alone.

I wish you well and hope that the next laser treatment will be the end of it.

  • Hi Marion ,

Thanks very much for your reply it was most helpful. I hope you don't mind me asking but how long was it from when they found displacia to you being diagnosed with laryngeal cancer? As this is what I think may be happening to me. They said inicially It could be treated with laser alone and after my last laser treatment yesterday that the return was on the surface only and did not warrant rdiotherapy at the moment and he had done deeper biopsys which he says hes sure arn't cancerous. This has as you know been a very difficult time for me as i'm so scared of loosing my voice or having to have a laryngectomy which I've been told is unlikely. i'm reassured to know that after your treatment your voice is now good. Thank you again and I hope you continue to do well. Andy

p,s gave up smoking 5 years ago and after this will never go back to it!

  • Hi Andy

That's excellent news! Hopefully that will be the end of it and I'm sure the hospital will continue to monitor you and reassure you that all is well.

I wish you all the very best for the future.

  • Thanks Marion same to you too.

  • Hi Marion, Andy,

Firstly thanks for replies, it is nice to be in contact with people who have been through the same as I am going through now.

I got my results back on wednesday and thankfully the dysplasia has reduced to moderate from severe and the consultant was happy with this and also that the time between the last two procedures was nine months. They were being done every three months before this and also he commented that the growths were greatly reduced and not as angry as the previous ones. Hopefully this is all a continous move in the right direction. I am not due to see the consultant for 3 months so hopefully nothing will return in this time.

My voice is varying on a daily basis with it either being good or quite hoarse. The only thing which is dissapointing is that I struggle in crowded places. Hopefully this will improve over the coming weeks. Is this something you have come across whist recovering? I am so desperate to get my voice back to a good level. It is functional but not ideal.

Once again thanks for your replies. I will keep you posted of my progress.

    Chris

  • Hi Chris

I'm so pleased that your results were good. The fact that your consultant is happy to see you every 3 months shows he's not too worried or he would want to see you sooner.

I used to panic about being in crowded places as you naturally speak a lot louder over background noise. I'm sure we've all been there - everything goes quiet and you realise you are shouting! When I'm in a noisy pub or if there's music playing, I get closer to the ear of the person I'm talking to, rather than shouting across people.

Try and look after your voice - drink plenty of water and try not to strain it. If you are very hoarse, then rest your voice for a couple of days and only speak when you really have to. Try and avoid long periods on the phone as you naturally talk louder than face to face.

As I mentioned before, my voice is better now after radiotherapy than when I had dysplacia but I used to vary between husky and barely audible depending on how much I had abused my voice over the weekend! Even now, my voice sometimes gets croaky and then I panic thinking the cancer's come back but I wait a couple of days and it goes back to normal again.

I wish you well and try not to worry (I know easier said than done)! Keep us posted.

    Take care,

    Marion x

  • HI Chris glad to hear your news, its good that you are being well monitered and it sounds like they are on top of the situation. Even though my voice is a lot better than it was I still sometimes have problems in crowds especially if there is loud music and I've had a few drinks my voice can sometimes go hoarse. Like marion I tend to get closer to people to and talk in to their ear if I can, usually my voice is ok when I get home. Sometimes it can be slightly difficult at work as I'm a personal trainer and work in a gym which can be quite noisy but again I find talking with your face to other person a little closer if they don't mind makes it easier. Real no no's for me are coffee as that tends to dry my voice out and it can become hoarse after 1 cup! Try and drink lots of water, and rest your voice as much as you can, I guess you know all this already. I have my next check up 10 feb so keeping my fingers crossed that all will be ok. All the best.

  • Marion, Andy,

Thanks once again for your repies. It is good to be able to be in communication with people who have had the same experiencies it really is a great help.

I will take on board both your comments regarding water as at the moment I am not drinking hardly any and am mainly on tea all day long with I am sure will dry it out. I have been advised not to drink in case it aggrevates the dysplasia until my next check up so I may go crazy and have a cordial or two next time I am out!!!!!

I hope that my voice returns to the levels both yours have as I am struggling at work at the moment as I have to hold a lot of meetings on construction sites so you can imagine the background noises I am up against. I think I have been pencilled in for some speech therapy in the the next couple of weeks so hopefully this also will have a positive effect. I think at the moment I may be expecting too much as it is only just over 3 weeks since my op so I would of thought they will still be swollen from the lasers.

Once again thanks and I will keep you posted.

  • Hi Andy

Just wanted to say good luck for your appointment on 10th Feb - what a coincedence, mine is the same day!! Let's post afterwards and compare notes?

    Best Wishes

    Marion x

  • Hi Marion,

Thanks for that and of course same to you, yes that is a coincidence. I always get worried about the check ups as I hate the whole experience having to wait for ages as the clinics always over run, I get really stressed worrying about the result but i'm trying to stay positive as the last result was encouraging so keeping my fingers crossed. Yes lets compare notes and hope we both get good news. speak soon.

  • Hi Marion, Hope all is well with you. Hope you don't mind me messaging you again but things have taken a bit of a dip with my voice. Up to about the end of feb my voice was really good almost back to normal then on my 50th birthday I threw a party and noticed that with all the noise my voice became very hoarse and it was a real struggle to talk to everyone, this had'nt been a problem since my last op but since that night things have become steadily worse and I now struggle to speak normally above back ground noise and seem to have no volume in my voice. Its really getting me down as I thought I had got my voice back almost to normal after the op and now I feel back to square one. I feel incredibly self conscious when I meet new people as they always ask whats wrong with my voice or they can't hear what i'm saying.

At my last check up they said the left side of my vocal chord looked sore and that they thought it was due to the scarring from the laser treatment, and they are keeping an eye on it to see if it changes. The consultant also said that my voice may never get any better and the more lasering I have the worse it will get, which was really depressing to hear. You mentioned that your voice came back to normal after radiotherapy. I just wondered did you have lasering done too? I'm thinking that perhaps if the cancer has come back I might be better off having radiotherapy as I can't bear the thought of my voice getting worse. I would love to have the hope that one day my voice like yours would improve but I know every case is different. Have my next check up on weds and am dreading it as I have a feeling they are going to say I need more treatment. Hope you don't mind me getting in touch but am feeling very isolated as friends have been good but they don't really understand how difficult this is for me. Best wishes. Andy

  • Thank you very much for your reply, had my last treatment in dec so they are keeping an eye on me monthly. will let you know how I get on. m thanks

  • Hi Andy

Everything was fine with me (thank god) and I go back in 3 months time. How did you get on? I was thinking of you today when I was waiting in the clinic.

Let me know, hope it was all ok.

  • Hi Marion,

Glad to hear all was ok with you. Surprisingly my result was a lot better than expected. He had a look and said it looked the best he had seen it and the area affected on the surface was greatly reduced and that he thought I was 95 per cent there with the treatment, although he still wants to see me every month. I was so relieved to hear that as my voice has been so bad this wk I was convinced it would be bad news. He also said my voice would improve in time and that my current hoarseness is probably due to me straining my voice, not getting enough voice rest and getting stressed about how I sound. I'm taking my self off to the lake district this w/e walking so I can rest my voice. Thanks again for your support and great to hear you are ok. Have a lovely w/e.

    Love

  • Andy,Marion

I have just read your posts and would like to say that good news comes in three's. However I can't. I had a check up yesterday (by coincidence) and mine has come back again. The consultant sais it looks beningn but needs to take it off and biopsy as per ususal.

Obviously I am a bit down and annoyed but am learning now that stress and worry is making it worse so I am trying to keep calm, but this is not easy with a new 1 week old babay in the house, but you can't choose when it comes or not.

By coincidence I was speaking to a friend of my neighbour who had exactly the same as me a few years ago, mine is caused by the HPV virus causing papillomas with dysplasia in them, and he had 12 op's in 4 years. However he now sounds great after the virus just regressed for no reason and he has had no signs of it coming back so there is light at the end of the tunnel. The hardest thing at the moment is the worry of keeping my Job as work is getting a bit scarce it it comes down to survival of the fittest!!!!

Anyway glad you both had good results and will keep you posted. I am having my op next Wednesday so fingers crossed.

    Chris

  • Hi Chris.

Hope your operation goes well there is not a lot that you can do about cancer all you can do is stay possitve, I have had 3 operations for throat cancer and a major op to get rid of it I am well now not back to how I was before but better, I did a abseil down a castle last saturday for Hope House charity only 65 feet, so best of luck , best of health and happyness for the future.

  • hI Zacker.

Nice to here everything has gone well for you, my voice took a lot to settle down I know it is hard not being able to speak when you want to. look forward to getting better and getting about, I come from the north east and think the lake district is one of the best areas to go. have a nice time.

    JOENINTY.

  • Hi Chris,

Sorry to hear your news, I know exactly how you feel as twice now I have gone for my check up to be told that my displacia had come back, the first time resulting in another operation. You just feel disappointed and frustrated, I think the thing to focus on is that this isn't an over night fix and its a bit of a journey on road of recovery and we are lucky that we are being constantly monitored so that hopefully we can avoid a more serious health problem which would be harder to treat. Having said this its hard not to worry and get stressed about it as your mind works overtime imagining all sorts of things most of which don't apply to us coupled with the difficulty of actually speaking properly. Although my result was good my voice has been terrible this week part of which I put down to stress and worry and having to talk at work etc when I should be resting my voice. I think its going to be quite a while before my voice comes back properly and I know it won't be like it was before but I live with the hope that one day it will and trying to stay positive, and not stress and worry, but it is difficult . I hope your op goes well on weds and I will be thinking of you, let me know how you get on and try and put it out of your mind this w/e.

    All the best Chris

    Speak soon

    Andy

  • Hi thanks very much for your reply and kind words. Love the lake district so really looking forward to it. Have a great weekend. Andy

  • Hi Andy

I'm so glad to hear you had good news at the clinic! I was wondering - have you been offered speech therapy? It might help you to use your voice without straining it and help you to relax, because when you are stressed your neck muscles tighten and it affects your voice.

Keep in touch and hope you stay healthy.

    Love Marion x

  • Hi Chris

I was so sorry to hear that you have to go in for another biopsy tomorrow. Please let me know how you get on. I expect you will have to wait for 2 weeks for the results though? Try not to worry as I have been down that road so many times and most of the time it was good news.

I'm keeping my fingers crossed for you.

    Take care,

    Love Marion x
