Psa levels changing

Two years ago I underwent a radical cystectomy, which included the removal of my prostate.  It transpires that I also had incidental prostate cancer. My scans have been clear for 2 years and I am under 2 London Hospitals. I have never asked about psa levels as my consultants have always said my bloods are fine. However, one of my Consultants mentioned that my PSA level had increased from .001 to .002, but was nothing to be concerned about and he would see me in 6 months. He was quite casual and I had never seen him before.

I know the levels are very low, but is this common, as I know that despite having my prostate removed the body still produces PSA  and therefore, levels can fluctuate. I am quite a nervous person and would appreciate a response.

  • Hi Ianw23. Welcome to the forum.

I'm not a doctor, but I had a radical prostatectomy back in 2010.  I've also been involved with my local prostate cancer support group, and as a result I've picked up a lot of background info.

After a radical prostatectomy, most consultants don't even start to worry unless the PSA level reaches 0.2 or maybe even higher.  In fact, most PSA machines can't measure values below 0.1, and so a value lower than this is usually reported by the machine to be < 0.1 (where < means "less than").

You can see that your figures are well below the threshold where one should be concerned about prostate cancer recurrence. However, I would ask if you're completely sure about the number of decimal places.  If the figure really is 0.002 or even 0.02 then you're well below the "worry level" of 0.2.  As you rightly say, the body does produce small amount of PSA outside the prostate which could easily account for this reading.

  • Hi,

Thank you for the message, it was very reassuring andcexplains a lot.  Unfortunately, they never explain these issues in full, so you are always able to understand the outcomes.

    Thank you yet again

  • Your own GP should have access to all the results via his or her computer so can always go there for a chat.  And if you haven't already, you should ask the hospital to send you a copy of any letters they send to your GP.

  • Hi ,my psa is 0.1 after 15 months post radiotherapy , and zoladez 3 monthly . I have 2 more injections which will take me to 2 years. My psa was originally 43 . Is this normal ?

  • Hi jonesylad. Welcome to the forum.

I'm not an expert - I've just been through this mill and picked up a little information.

A PSA of 0.1 is definitely a good result.  Don't be too alarmed if it rises and falls on each reading; as long as your consultant isn't worried, then you shouldn't worry either.

  • Cheers, I am hoping after last injection I won’t have to wear a bra lol

  • Well, you could always choose a nice lacy one with bows to match the colour of your eyes

  • Yep there is always that. I was wondering about the weight gain lot of people have different views. Little price go pay though for still being here.

  • I don't have any experience of hormone therapy (I was lucky I didn't need it), but there are a couple of men here who do.  I'm going to name check them here and maybe they'll jump in.  They are [@Billygoat]‍  and [@woodworm]‍.  Alternatively, you could post a new question (be sure to mention prostate cancer in the question).
