Decision to refuse treatment & advanced care planning

I have  received these forms from my palliative care team and wish to put in place the documentation my family may need for guidance in the future. 

Does anybody konw what kinds of treatments are refused in specific circumstances? I think when the prognosis for Lung Cancer may be poor this is something we need to be informed about.

    I can share what hubby chose to put on his forms.  He had lung cancer as primary, secondary adrenal gland, bone cancer to back and sacrum and mets to brain.  He as an ex nurse had some medical awareness
    His values and wishes
    To stay at home as long as possible unless pain too extreme and cannot be managed at home.
    Any signs of bedsores at home -cease home care.
    I would prefer to have a better quality of live for a shorter timeframe.
    Under binding refusal/s of particular health care
    No cpr
    No ventilation of life prolonged by artificial means and the medical staff determine that issue.
    I do not want to be fed by tube
    I may be offered food and fluids by mouth
    I have just copied this from his forms and hope it may assist you in choices
    We are in Oz.

    That is similarto what I have drafted onto my for  which is currently in a handwritten draft format I have a adenpma carcinoma of the left lung which has spread to my bones, lymph nodes and cancer in lower spine

    - I have worked in clinical reearch and the NHS for many years but have still interested in the wording which you husband has used wihich is more precise that what I have drafted

    Our son (26 yrs) and I were appointed substitute decision makers but at the end of the day as everything was documented we didn't have to make any decisions.
