Ache in testciles and perineum

Im 62, and up to now ( as far as I know ) in good health. Last week I started to feel an ache in my testicles, perineum and anal areas. Its not constant, sometimes it seems to go. I am doing number 1's and 2's fine ie no blood etc. I'm eating fine and generally feel well.

I had my prostate checked about. year ago and the GP said it was normal.

When I spoke to the GP on the phone he said take ibubrofen for a few days. I did and it hasnt really made much of a difference. I am probably urinating more than normal buts that because I am drinking 4 or 5 pints of water a day to try and flush anything out !.

I dont have a family history of cancer of any sorts although they did find some cancer in my dad when he was about 90 but it didnt kill him.

Am I over worrying ?

I did wonder if I had damaged a nerve as I have been doing lots of DIY the last month. Woudl a physio be able to spot anything

Any help/suggestions would be very welcome

  • Hi smallzoo. Welcome to the forum.

I'm not a doctor and I have no medical qualifications so use your own judgement with anything I say here!

I've had on and off testicle pain for the last 40 years.  Sometimes the pain has been caused by an infection with the tongue twisting name of epididymitis which has required antibiotics.  But sometimes it comes for no reason, antibiotics do nothing, and then it settles back down again. I had a clear ultrasound with nothing found a couple of years ago.

You also mention the pain has spread to your perineum.  This could be referred pain from the testicles, or it might be prostatitis.  I know you had your prostate checked a year ago, but I think it would be wise to have it rechecked - maybe get a PSA blood test.

Or it could also be a urinary tract infection. There is some evidence that cranberry juice can flush out bacteria from the urinary tract.  Even if it doesn't help, it won't do any harm and tastes very pleasant so you might like to try it.

These days doctors seem very reluctant to prescribe antibiotics which might be why the GP is suggesting painkillers to see if it settles down again.  If ibuprofen alone isn't helping, you should also be able take it in combination with paracetamol - just as long as you don't exceed the max daily dose for each painkiller.  If in doubt, check with your chemist.

I'm so used to having these symptoms that I generally leave it for a couple of weeks before telling my GP - unless the pain is very severe.  I think you should give it a week to see if it settles down and if not, then phone your GP again. Don't forget to ask for a PSA blood test.

  • Thanks for that. Sorry to hear you have had problems for so long. My pain is not severe more like a mild headache but around the testicles/perineum/bottom area. The reason I though it might be a nerve or muscle is that my bum cheeks feels like they've been doing a lot of exercise and the muscles hurt a bit.

I'll chat with my gp again and get him to check the prostate and have a psa test just in case

    Cheers

  • I recommend all men over 50 have their PSA checked regularly.  I had prostate cancer at age 56, but because my PSA was regularly checked it was caught early and treated by surgery.  That was 10 years ago  and I'm still here to tell the tale because I asked for the test!

  • Some Great advice. I'm currently experiencing these kinda pains, but for no apparent reason.

Will keep with the pain killers.
