I guess it can mean different things in different NHS trusts. Generally speaking, it usually refers to the group of different clinical professions helping you. The alternative is your consultant working alone. It doesn't necessarily mean your clinical needs are hugely complex; it's just a good idea and good practice. That's all.

Its a team of different experts looking at your case and how best to help you ,everyone gets this before they decide the best plan for you Callum .
Ask a nurse or doctor to explain it to you. You should be told which kinds of health professionals are in the care team, and the reason why they are in it. I've always found it quite confusing myself
