I think most of us do this to a lesser or greater extent. It spares the other person an often awkward conversation and leaves them going on their way happy in the knowledge that you are not suffering to any degree. My quip is usually I'm alright or I'm fit and well thanks. Im a bit more direct with people close to me but still temper my reply to a more positive note. I dont have any problem doing this and it helps me keep in an upbeat mood.

If its taking its toll perhaps its time to be a little more open or direct with someone you trust or here
The nurses on the helpline are good to chat to too
Its okay to say Im not okay thanks and yes it might feel awkward but so what ? Be you and be kind to yourself other people might be stronger than you imagine
