Hi Mouse67 and welcome to the forum though sorry you have to be here and your dad has cancer, A fast track discharge is done when the patient, your dad, has maybe a few months left and has told the hospital he wishes to be at home for the end. He will get a continuing healthcare plan from the hospital for when he's home, but i suggest you call your dad's GP and get a referral for the hospice palliative care team and a district nurse for the best homecare, Mouse I have prostate, bladder, rectal and lymph cancer and have a district nurse and access to the hospice and the team and i can't thank them enough, If i can help in any way, just ask, best wishes to you both.
