can someone please offer me some advise - PSA results

My dad was diagnosed with T3a N0 M0, Prostate Cancer about 1 year ago, he also has T3 Lung Cancer! He began with Injections his Gleason score was 8, his PSA was 10.1, he finished an intense course of radiotherapy in December this week we went back to the hospital for his first appointment after radiotherapy, they had taken no PSA reading so had this done on the day!! But consultant said that she didn't believe there would be any problems and would see him in six months, however today he received a letter to say that his PSA was 6.3 and was elevated more than what they would expect post treatment and he has to go back for further tests in 2 months!! Is this normal for this too happen or is it an indication that treatment hasn't worked?? I can't seem to find enough info any info would be great thank you

  • Hi debz76.  Welcome to the forum.

    I am not a doctor. I am a prostate cancer patient who had a radical prostatectomy in 2010. I've also been involved since then with our local prostate cancer support group.  This means I've picked up a lot of information along the way. 

    I'm not an expert on what happens after radiotherapy, but I do know that the PSA does not drop to its lowest level until 12-24 months after treatment.  In the first few weeks after treatment, there is a rapid drop from the pre-treatment level.  I've managed to find a couple of graphs of "typical" PSA values versus time after treatment, and it's quite clear that the first few months are going to subject to a lot of uncertainty due to the shape of the curve.  Your father's PSA is somewhat higher than "typical", but not outrageously so.  A curve might be "typical" but that does include a lot of scope for individual variation. There is certainly no immediate cause for concern, particularly if the next reading (after 2 months) shows a lowering.  As I said earlier, it can take up to 24 months (or even longer) for PSA to reach its lowest value.  I know it's difficult, but try not to worry.

    Note that the decline in PSA after radiotherapy is completely different from that after surgery.  After surgery, PSA drops like a stone and should be < 0.1 (less than 0.1) six weeks after surgery.  Don't get these confused in your mind. 

    Regarding PSA tests. I always arranged with my GP surgery to have a PSA test 7-10 days before my hospital appointment.  This mean that I could pick up the result a few days before the appointment and if necessary I could tell the consultant the result!

  • I was diagnosed with stage 3 prostrate cancer 2 years ago had radiotherapy and prosap injection my psa came down from 120 to .6 however 3mth ago it raised  from .6 to 1.2 and was doubling up every 2 weeks the oncologist has now prescribed zitaga and persodene my psa went up to 6 I was at the hospital this week and it has come down to 5 I now only have to go back in 2mts time and they are not concerned and said it is tumior flair 

    I am not concerned as I believe I am getting the best treatment they can offer and cannot waive a magic wand I BELIEVE WHAT WILL BE WILL BE I don't dwell on the condition I accept life on life's terms and the clock of life is only wound once
