Immunotherapy treatment and MS

Having had two surgeries and one round of radiotherapy it is now being proposed that I should have immunotherapy with cemiplimab to treat the skin cancer that seems to keep travelling around my head neck. My concern about this, quite apart from the possible "ordinary" side effects that goes with it, I also have Relapsing Remitting MS which had been being treated with Ocrevus until my cancer was discovered when it was stopped. It seems the makers of Ocrevus are concerned about patients with cancer. Now I'm reading that the manufacturers of cemiplimab were concerned about patients with neurological problems that they were excluded from any clinical trials. I really need guidance here about the wisdom of going ahead with this treatment or going back to the oncology team for their next best suggestion.

    I am sorry to learn that you need further treatment for skin cancer. This must feel very difficult, and, understandably, you have concerns about starting cemiplimab, especially with your history of relapsing–remitting MS and previous treatment with Ocrevus.

    Your oncology team is best placed to talk this through in detail, as they know your full medical history and the specifics of your cancer. It may help to ask them about the expected benefit of cemiplimab in your situation, how they assess the risks given your MS, and whether they can discuss your case with your neurologist so a joint plan can be agreed. The doctors will always weigh the potential benefits and possible risks, and you can certainly ask whether there are alternative options available.

    If you have a clinical nurse specialist (CNS), they may also be a good person to speak to for further support and to help you explore your options.
