Delay in treatments for stage three breast cancer - will it have spread?

I had a 6.5cm lobular breast cancer tumour removed in a single mastectomy 17 weeks ago but only started radiotherapy on 31st December - my oncotype dx test score was 9 and I’m 55 years old and perimenopausal. I had cancer in my sentinel lymph node and so had all of the others taken out in a second operation but all 29 were clear. As yet, I’ve not been given any hormone tablets but I believe I’ll be given Letrozole and Abemaciclib after three weeks of radiotherapy. My big worry is that there has been a gap of three and a half months without any adjuvant treatment and I need an honest answer as to how likely it is to have spread please. Thank you.

    After breast cancer surgery, it's quite common for there to to be a pause before radiotherapy and hormone treatment start. This allows proper wound healing, radiotherapy planning, and your full pathology results. The timeframe you've waited does not mean the cancer is likely to spread. Breast cancer, especially hormone positive lobular cancer tends to grow slowly.

    Your Oncotype DX score of 9 is very low showing that the cancer is slow growing. Only the sentinel lymph node contained cancer, all other nodes were clear which is reassuring. Your cancer has been fully removed with surgery. The treatments you are now having are to reduce further risk, not because spread is suspected.

    Starting letrozole after radiotherapy is normal ,and hormone therapy works over years not weeks. With all this in mind, if microscopic cells were ever present, the treatments you're receiving are designed to deal with them.

    The best person to advise you further on all this is your specialist team. If you have a specialist breast nurse (CNS) involved in your care care then perhaps they can provide you with further information. You often have to call and leave them a message and they will get back to you.
