High risk endometrial Cancer - options

A transvaginal ultra sound has shown a 1cm polyp, free fluid in my uterus and a 20mm lining thickening. The consultant wants to do a hysteroscopy under GA in 3 weeks. From what I’ve read a thickening of 20mm of nor is automatically considered precancerous. I’m 52 post menopausal, obese and have PCOS the reason for referal was my new GP noticed I had been given the wrong type of progesterone for last 6 years as part of hrt putting me at higher risk. My tablets were changed and I had 2 heavy bleeds days apart My question is due to my anaesthetic risk and prognosis can I just cut to the chase and go straight for a hysterectomy? I don’t want to be at risk twice for what is an inevitable outcome without further delay. Do I have this choice? I have moderate hearing loss and find any hospital experience totally nerve racking, masks and strong accents make aiding my hearing with lip reading impossible, even more so when coming out of anaesthesia (my previous 2 GA experiences I’ve had to be given sedation post op due to distress/aggression I experience) I feel like the hysteroscopy is added risk and delay but I’m expected to just go along with it.

  • Hi Mickymac, I had the hysteroscopy at the beginning of November,  I didn’t have a GA, I had it without it,  it was a bit uncomfortable but I just  sat there and watched it on the monitor, what they were looking at. They had taken a scraping for a biopsy, I got both results, within  4 weeks due to a backlog, I have endometriosis , and I have endometrial cancer they caught my cancer very early. I met the surgeon who is going to do my operation, I am having a full hysterectomy, to have my womb, my cervix and my ovaries removed. I am going into hospital on the 16th December to have my operation. I must admit I am very scared, but at least I have the support of my family and friends behind me to wish me luck.

  • The only way to know if your consultant is prepared to skip the hysteroscopy is to contact them and tell them about your preference. I think they assume most people would rather not come under the knife if they can avoid it but many women I've spoken to would opt for a hysterectomy for various reasons especially after menopause.

    Good luck, I hope you get an option that suits you best and that it's not actually cancer even if you opt for the hysterectomy without the hysteroscopy because it's always better for it not to be cancerous.

  • Wishing you the best minniemouse, can I ask if you know why they decided on removing cervix as well ? Glad you’ve got family and friends behind you, I will too if I need it, but what no one can help me with with all these appointments etc is being self employed it will wreck my business as clients are hard to gain but jump ship all to easily if you don’t meet their needs. 18 years of creating something that could be ruined in a few months just as youngest has decided to go to uni next year, Never thought we’d be reduced to baked bean suppers again, but can see if happening as will be just outside parameters for financial help.never a good time but this is worst. I hope it goes smoother for you.

    I am sorry to hear about your concerns. it is difficult without being involved in your care to know what your doctors will say if you ask for an immediate hysterectomy.

    The doctors normally would like to assess a woman's womb before proceeding to this surgery.

    It is really only when the pathologist looks at the cells (from any biopsies that are taken) under a microscope that they can say for certain whether cancer is present.

    If the doctors are considering a hysterectomy they will also want to assess your general health and your anaesthetic risk if you are over weight as you say.

  • Morning Lyns21, I’m glad I had the hysteroscopy. Because without the diagnosis and the biopsy, that they had found the cancer, and I had left it alone, they said that I will not be here in 2yrs time , and it would be too late. I couldn’t go through this with chemotherapy. It was bad enough to watch my mum having chemotherapy. My cancer was a low grade, and that is why I decided to have all the tests and have the full hysterectomy. You don’t have long scars anymore, it is all done by laparoscopic robotic keyhole surgery, where you stay in overnight and you recover quickly. Please have all the tests, biopsies and the hysteroscopy. It will save your life.
