Breast cancer

Had a mastectomy 21 years ago with a reconstruction from my stomach muscle. Last August I found a lump in the same breast. I have had the lump removed but have been told I will need another operation to remove more cancer having a total mastectomy so removing my reconstruction. After I will need radiotherapy. They have said I might need Chemotherapy. Do you always have to chemotherapy?

The pet scan showed it had not spread

    I am sorry to learn that you have to have more treatment after all this time. 

    Chemotherapy isn't always recommended in early breast cancer, it can depend on features of the disease and often the results of the tissue examination after surgery feed into the decision.

    After or sometimes before surgery, a body wide treatment is generally recommended, so if the cancer isn't hormone sensitive and hormone therapy isn't therefore indicated,  the specialist team may want to consider chemotherapy. But sometimes hormone therapy and chemotherapy are both used.

    If chemotherapy is suggested you should have the chance to talk it over with the specialist and be told the pros and cons before you have to decide about it.  But for now it sounds like surgery is the next step and I hope it isn't too far away.
