About Liz-up-north

Hi, my mum has unexpectedly been diagnosed with lung, liver and bone cancer 5th Aug 2024. She want in hospital with federal decline. I live up north and she lives in Bath area. Struggling with hospital discharge team to get clear idea of discharge plan, carnt seen to get straight answer or speak to them. 

What should I be asking, Dr's, discharge team, what do I do with flat if she goes in care/ hospice, do I let bank know... Practical not emotional help needed please... She is to frail for treatment but they have not said end of life something like best care! Any advice?? Thanks
