7 weeks pregnant, ?miscarriage, abnormal smear & HPV+

Hello

Hoping somebody may have been in the same situation as me as I am past myself worrying.
I'll start from the beginning!

My husband and I have a 6 year old boy and I have recently found out I am pregnant. However at 6+5 I started spotting and at 6+6 (29/6/2021) at a private ultrasound scan no fetal heartbeat was detected so we were referred to the early pregnancy unit. I was seen there on 1/7/21 and was found to have an inconclusive scan and have a repeat scan on 11/7/2021 to either confirm a miscarriage or confirm a continuing pregnancy, which I doubt is the case. My spotting has continued and I have had mild abdominal cramps.
I had my smear test 6/5/2021 and today received my result which is - positive for HPV as well as some borderline cervical changes. With my results letter I also received my appointment for a colposcopy for 6/7/2021, which is next week.
I called the colposcopy clinic and explained my situation and they are happy to see me still as colposcopys are safe in pregnancy.
However I am beside myself to the point of nausea with worry as I feel that the colposcopy appointment being so soon must mean that they have rushed to see me and this therefore must be bad news.

Has anybody been in a similar position please? Any words of wisdom or shared experience would be so appreciated.
thank you! xxx

  • Hi  jademp,

I am not in the same situation as you but i have just had a colposcopy and lletz treatment for ?grade 2/3 precancerous cells and had the appointment within a week. I had high grade cells and positive HPV found (this was after 2 normal smears in the past!)

I think when HPV is found and any changes they invite you in quickly within 2 weeks to check. The smear itself isn't diagnostic it's just screening to see who needs a diagnostic test. I too felt quite sick before. I had the colposcopy on Thursday and I find smears quite uncomfortable but honestly I wouldn't even say the colposcopy was painful, same as a smear really.. the microscope is on the outside. The most painful part for me was when they removed my coil as I had one in! I did need treatment to remove the cells called a lletz where they burn them off, i had it under local anaesthetic at the same time and it wasn fine. I am waiting for biopsy results now to determine exactly what grade it is and if it's all gone.

My friend had more borderline changes and just had the colposcopy and a biopsy to check the grade. It was a lower grade so she just needs regular smears. I have another friend who like me needed the lletz a few years ago and had been fine since. They can do the colposcopy when your pregnant but I don't think they can do the lletz.

Hopefully yours will be a low grade you just need some survallience to check it's not getting any worse or hopefully it goes on it's own.

All the best for your colposcopy and sometimes like me you leave not knowing exactly whats going on as they still have to wait for the biopsy (mine takes up to 6 weeks they say). The consultant said if mine is grade 2 they have possibly over treated me (as they could do surveillance with grade 2) but there is no way knowing without sending a sample to the lab

  • Thank you so much for your reply Jess.
I'm glad to hear your procedure was smooth and that they have been able to perform the LLETZ. I hope your results are back sooner rather than later

My colposcopy was yesterday which was straightforward and not uncomfortable. However she did find that the changes on my cervix were moderate changes rather than just the borderline changes detected in my smear. Unfortunately she wasn't willing to do the biopsy until there is more clarity on how my pregnancy will progress, so I'm to call back on Sunday and inform her.
If the pregnancy is viable, she said she will arrange another colposcopy when I'm 28 weeks and then a biopsy/treatment 3 months after the birth.
If the pregnancy is not viable then once the miscarriage management is complete then she will arrange a biopsy after that.
However if the pregnancy does continue I think I will ask for a biopsy at the earliest opportunity rather than waiting months for the cervical changes to potentially get worse.

Hoping all stays well with you xxx
