Bleeding for 6 weeks

I have always had heavy periods and started from the age of 13, from this I was put on transamatic acid for up to 6 years, i had to stop them as i felt I was on them for to long. Periods were still heavy but the last 2-3 years I had a number of problems with a swollen abdomen and constant water infections, pain throughout sex, after sex and  I had spots of bloody discharge during and after this, feeling exhausted and fatigue all the time. I am currently on the reguvidon pill and have been for a year an a half now. I have had a number of ultrasound scans and internals all which they have said there is nothing to see. The last 6 weeks I have been non stop bleeding, lower back pain, slight abdomen pain and feeling very fatigue all the time, this bleeding has also happened throughout me taking my pill when it was not the week I should of had a period. The doctors have recently just sent me away with antibiotics due to another water infection that I felt no symptoms for at all this time. I have been put on antibiotics for the 6-7 water infection over the last 2 years. I have had a 3rd ultrasound a few days ago which resulted in all clear. I have asked to be referred to a gynecologist but my doctor seems to be avoiding the referral or a smear test due to my age, I have just turnt 24. I am very worried about this and as currently bleeding for so long it is a very unusual thing for me it's making me very emotional and stressed with all the current problems over the last few years I need some advice, can someone please share some advise to help me in a path to take to get some answers?
