Total Duct Excision - What can I expect afterwards?

Hi, my first post and I'd like to see what everyone's experiences have been.

I've been having spontaneous clear nipple discharge since August and after referral they suggested a single duct removal. Ive just come back from the consultant today and they have decided to do a total duct excision now which has thrown me a little, not sure why...suppose it just seems like a bigger thing and I hadn't prepared for this.

One of my questions is what is a realistic recovery time and apart from the scar, does it leave the breast looking less full? They did mention lipofillers today.

Im 41 and have 3 young children, and probably distracting myself with details like these rather than what the results might be, bit thought this looks like a good place to get advice from others that might have went through this.

  • Thank you for sharing these experiences. All of you are so brave and it's been so hard to find info around this. I'm due to make a decision whether to go ahead in Jan 2023 for the op to have right breast total excision. I declined this month as I felt I didn't know much about this. Like you said, your mind goes blank and everything becomes a blur in the consultation. Now I've read these responses, I feel so much better and clearer to go ahead.. So thank you for this. I was worried about how it looked after, the procedure and recovery.

    I've had spontaneous clear nipple discharge since March 2022, it turned bloody in June 2022. I'm 31, a mom of a 4 year old and married. I've had 3 ultrasounds nothing came up. More recently I keep getting pain in my right armpit. Bloods checked in May nothing? It's so strange 

    I was told they wouldn't do a mammogram or mri. BC is on my mom's side, Aunty, cousin carry the Brac 2 gene both has double mastectomy . Great grandmother died of BC. Paternal grandmother ovarian cancer. I'm due to complete a form for genetic testing.

    I'm sorry to hear you're having to go through this procedure, but I promise you, it's much better than you imagine it's going to be. I had my op two weeks today, and the dressings are off and wound is healing really nicely. I have a scar around the bottom of my nipple and it's still quite tender to touch, but I didn't have much pain at all with it and I was really surprised how small the scar really is. 

    I had a bloody nipple discharge for a while and my consultant said this op would be the best way to stop the discharge, which it has done. I get a my biopsy results next week, but my consultant said she's not worried about them, so I'm taking that as a hopeful positive outcome. 

    So honestly, don't worry about it at all. The breast surgery team are the nicest people you'll ever meet and they look after you so well, both pre and post op. So you'll be in really good care.

  • Just popping on as I was the original poster and wanted to reassure you. 

    Its almost 3 years down the line and my scar has completely healed and ive had no discharge since. Full sensation in my nipple too.

    I must admit the op floored me but I had an infection afterwards that delayed recovery.

    Boob is same size as other one and you would never know there was surgery.

  • Hello I’m new here but was quietly following the discussion in the background.  I had total duct excision exactly 2 weeks ago. It all happened so quickly I barely remember anything.  I started having yellow discharge on one side in August the in seemed to stop but if you squeezed you could hit someone in the eye with the force of the stream that came out. So I thought I’d better get it checked out. Within 3 weeks I had clinic appointment had a mammogram and ultrasound and they were clear. I was told the surgeon might want to see me for further tests but there was a waiting list. Three days later I had a phone call telling me an appointment had been made for my operation. I was so gobsmacked I forgot to ask what it was for! You start to think the worst. Anyway op done breast is bruised but minimal pain and it looks fine. follow up for results in another 10 days. I’m pretty sure now it’ll be fine. I’ve been pretty tired (I’m 65) but drove and did shopping on my own today   Have to start some time.  Best wishes to everyone on the same journey

  • Hi, It's great to be able to speak to all in the same circumstances. I had total duct excision 6 days ago, it all happened very quickly after I started bleeding from one nipple. Four weeks later and here I am. I've got dressing on and my breast has gone from purple to yellow with bruising and my nipple turned a very dark purple again colour coming back. No sensation though. Pain wise it's been really insignificant it's more the anaesthetic that knocked me out for 2/3 days. Oh if you get the chance of a wireless front fastening bra take one, protects and supports your breast and so comfy too x I get my results on the 28th December x 

  • Thank you for posting this information.  I am having a total duct excision in 5 days and still don't really understand what is happening and I'm feeling really anxious and insecure about it.  I tried posting on here, but was just feeling really exposed so this is really helpful.  I've had all my pre-op but no ones really told me what the surgery is or how it's done.  It's good to see that there are some positive outcomes.

    This is a great platform to talk to others who have beeb through the same thing as you're going through, so if you have any questions don't hesitate to ask us.

    In my experience, the procedure was quite straightforward. I now have a 180 degree scar around the bottom of my nipple and that's the only thing that shows I've had the op. It was a lot less painful than I anticipated and like someone else I this discussion, I only took paracetamol for a couple days afterwards as I was advised to. It's tender for a few weeks after the op, but not painful. From what I understand, the op involves removing the milk ducts which are quite small, so you don't lose any fullness in the breast. 

    I hope everything goes well for you. Try not to worry too much - it will all be over really quickly. 

  • Ive been reading this since finding out in november i needed a right total duct excision and wasnt sure what to expect. Was concerned about having a general anaesthetic as not had before and im diabetic. Anyway had my surgery at 6pm tuesday and was home by 11pm. I feel good, some discomfort but other than that so far so good. Have taken some paracetamol but not much. Ive been signed off for 2 weeks which seems quite long but looking at other posts sounds similar. Im living in a sports bra which is so comfortable.
