Subungual melanoma (SUM)?

I’m a 51 year old mixed race male, reasonably fit and not overweight. About a month ago I noticed two bruise like marks, one on each big toe. There has been no trauma to either toe and all of my shoes/trainers are a good fit. For about a week I ignored the ‘bruising’ and was determined not to research anything. I scrubbed my toenails in the shower hoping and thinking it may be sock dye. It wasn’t.

I eventually researched the symptoms and saw a type of cancer called SUM. I read that dark skinned people between 40-70 are in the risk category. I made an appointment with the doctor who examined each toe under a magnifying glass. He immediately made a referral and I received an appointment a few hours later with a local dermatology department. That appointment is tomorrow.

I’m keeping an open mind for now and until an actual diagnosis is made. That said, in the back of my mind and with the risk factors identified, I am bracing myself for the worst. It’s slightly unnerving because I don’t have any other health concerns but can only hope an early recognition will confirm or negate it being SUM and I can deal with it accordingly.

I have nothing but praise for my GP, his bedside manner in seeing me, staying non committal and calm and making the referral without delay was truly first class. He’s a credit to his profession.

    It sounds like your GP is on the ball - if only all could be the same. His quick referral is exactly what NICE says should be done and that speed will help should a diagnosis of SUM be made.

    SUM is one of the rarer types of melanoma and I must admit that I have never heard of someone having it occur in two places at the same time (both your big toe nails). That said, melanoma is an unpredictable cancer so the dermatologist may want biopsies from both of your nails to rule it out and you will then have to wait a couple of weeks for the results. You will know more tomorrow so I wish you lots of good luck and please let us know how you get on,

    I am a 33yo Caucasian woman, with a single, growing, pale brown line growing darker and wider, up my thumb nail. Had my first derm referral (which involved taking pics) in Dec and due to having a baby in Feb, only had my follow up last Wednesday.

    The result of that is, since there has been clear change, I’m required to have a biopsy. This will be a full removal of the nail and some skin for testing. Awaiting this appt. 

    Feeling nervous too, there seems to be very little information out there regarding the procedure, recovery and IF there is melanoma, treatment and general facts about it. 

    Do you have long to wait for your appt? 

  • Hi,

    I got my appointment the same day as seeing my doctor. The appointment to a dermatologist was set within a week (tomorrow). SUM, if that’s what I have, so far undiagnosed, is extremely rare. There are a few medical articles about it online that give a good starting point for, diagnosis, prognosis and treatment.

    I’m nervous too but have to wait and let the professionals do their work.

    Thanks for enquiring. I’ve been told it’s not SUM, relieved beyond words. Despite not recalling any trauma I was told the marks are in fact trauma to my toes.
