Biopsy?

It has taken 6 months to get a diagnosis in my ear of a canal cholosteatoma with exposed bone. I had a CT scan which showed widened ear canal from the growth which has eroded bone. Original diagnosis was chronic otitis externa with exposed bone from ear trauma ie cotton bud and a cyst. Then it was suspected necrotising otitis externa when i had a servere bacterial infection. 

My ENT surgeon says he cant operate due to blood supply and nerves as would require a skin graft. Seeing him again in 4 months. I have regular 6 weekly microsuction ear cleans to manage it. 

I cant understand why the skin is in such a terrible state. Research says it is widely operated on. The ear is painful, itchy and weeps inside. I am only 52, no diabeties. I cant get any water in for risk of infection. At one point ENT doctor said it was the worst ear he had seen.

I cant help feeling something more is happening. Research says ear cancer can be misdiagnosed as a canal cholosteaoma as CT scan cant tell difference between them.

Should I ask for a biopsy to be carried out to check? I dont want to come across as a smart alec with the consultant. 3 diagnosis so far cant help wondering whats next. I am at my wits end with the pain, discomfort and continual appointments.
