Extensive skin rash and severe itching non responsive to standard treatment

Last yr I developed a skin rash that lasted for 3 months before settling down. I've now got it again and this is the third monthand it's just spreading. I've seen my GP 3 times. They've tried anti fungal, steroids cream and I've tried various home remedies. The rash started on my chest spread to my kneck and now is mostly on the arms and belly with the odd spot elsewhere. It's bumpy, red and the itching, while not constant, is intense and disruptive.

I have vascular liver disease (liver blood  tests normal range) and mesenteric panniculitis, for which there is a possible link to increased lymphoma risk. I've read medical authority suggesting that monitoring for lymphoma is advisable with MP. I have never been regularly monitore. My GPS have been poor. I've sought a dermatology referral and the hospital have rejected the referral back for a  local dermatologist. No sense of urgency when I press my surgery. My last request for another Dr appointment has been triaged on a await and see basis. This happened once before when they 'mistakenly' cancelled my request.

I have been very tired during the day - falling asleep mid afternoon perhaps for an hour or two. 

What to do? One gp said go private. I will now ask for an urgent referral as this spectre of possible lymphoma is back on my radar, but I have little confidence in my doctors and the dire state of the NHS. GPs often seem resistant to patients who try to direct treatments and investigations in my experience. 

Ideally I would have a quick referral for possible lymphoma but how do I achieve this with so much resistance? Your thoughts would be appreciated. My partner had a rare lymphoma over 10 yrs ago. At the time, the system treated his worsening problem as trivial by many drs and only when we leap frogged local dr's and dentists did we eventually get a diagnosis. So, I have experience of the terrible inertia and poor diagnostics in the NHS. And now I'm concerned I will fall foul of it myself.

I live in South east London, NR crystal palace, so have easy access to various medical centres potentially. Do walk in centres exist for dermatology. Somehow I need to get beyond the unhelpful triaging at my local level. Any help or suggestions pls?

    I'm sorry to hear that you've had some ongoing health concerns and that despite multiple visits to your GP things haven't improved and you're feeling frustrated by the system. 

    I don't know if you've considered changing GP surgery. If this is something that you want to consider then you can find information about how to do this on the NHS website. Alternatively, you may want to make a complaint to your current GP surgery about the care you feel that you've had, or not had. Information about how to raise concerns about a GP practice can be found on the Parliamentary and Health Service Ombudsman website. 

    You mention attending a walk-in centre. I don't know if there is a walk-in centre local to where you live. The NHS website does mention skin infections and rashes as something that an urgent care centre may be able to help with so this may be something you want to explore. 

    I do hope that you're able to get some answers soon regarding this ongoing issue and that things can be improved for you.

  • Thanks for your reply, Jenn. I've left a message for my GP practice manager to call me and will follow up with an email if I don't hear today. I'll keep the formalised complaint route in mind, but don't like having to go down that road. Didn't really know about urgent care centres. I've located those nearest to me and, if I've no joy by Monday, may give that a go. Don't know if they can organise tests and referrals. This week, despite asking for an appointment to consider an urgent referral, I have had to keep chasing my surgery and all I've got is a triage response that reception will get back when there is an appointment available.

    Spoke to private dermatologist clinic today to see if I could have a first consultation privately then continue with the NHS. Their earliest date 7 June and, if I want to continue with the NHS, they would have to refer back to my GP who would have, then, have to submit a referral as appropriate. So, even if I pay for a private consultation, I've serious doubts how that will help. It doesn't sound quite right that, after seeing a doctor who works as an NHS consultant during a private consultation, that she couldn't refer me directly for tests etc on the NHS. Is there a workaround to this - it would be wasting time again with my GP surgery and I've no idea who slow they might be.
