hello everyone

i have been dealing with paget's disease symptoms for well over a year now , in my left nipple . it hasn't responded to cream treatment and i have a gps appointment with the gp whos seen me since the beginning tomorrow so im going to ask about potentially getting tested . i am extremely worried about it , my maternal grandmother has had breast cancer and i know that paget's disease affects the nipple whereas eczema affects mainly the surrounding surface tissue . i also have a flattened nipple , and the discharge was constantly leaking from it to the point where everyday i come home and it hurts to take my bra off as its leaked and dried , attaching to the fabric of the bra . i am worried to bring this up to my gp as

1 ) i have anxiety , so i dont want her to think im overreacting / immediately assuming the worst

2 ) i have autism + depression , i have history with non compliance with treatment

3 ) my most worrying point - i am young . i have all the symptoms , i match the physical presentations ( ive matched myself to photos ) , i match the difference between paget's and eczema , but i do not match the age . i am scared that i am overthinking massively due to my health anxiety , and that it is simply an infection or eczema . 

does anyone have any advice ? i hope this doesnt come off rude and like im way overthinking it to push into a space where i dont belong . if i do manage to go ahead with testing that rules it out i will update .
