I'm so sorry to hear about your Dad's diagnosis. It's obviously a very difficult time for you all and it's understandable that you're feeling overwhelmed by things. 

Presumably, the hospital team will have been in touch with your local palliative care service or Macmillan team and they should be in touch soon. If you've not had any confirmation about things then I'd suggest getting in touch with your Dad's GP as the first point of call or giving the Constulant's secretary a call to ask if they have referred your Dad onwards for support.
