Tongue Cancer

Hello,

Hoping someone can offer me some information and advice. My mother in law has been told that she has tongue cancer. From what I understand she had her biopsy today and was told the lump on her tongue was cancerous. Can the doctor tell straight away that it is cancer because I thought the lump had to be sent away to a lab? Also what happens once the diagnosis has been confirmed? I assume it will be staging next with scans and further tests? Would be grateful for any information.
