Anyone had DCIS after breast cancer and mastectomy?

I had triple negative breast cancer that had spread to lymph nodes 2 years ago and had a double mastectomy and lymph node clearance. I've just found out I've got DCIS near the original lumpectomy scar. (I had lumpectomy originally and mastectomy later) 

I just don't get how I have any ducts left? 
Anyone else been in a similar position?
