Breast Cancer - Further treatment - Tamoxifen or Ovary removal...

Hi,

The story so far: My Girlfriend found out back in February that she had Breast Cancer. Luckily she caught it early. It was Stage 1, Grade 2, less than 2 cm, they removed it, and some lymph nodes. The nodes were clear, as was a subsequent CT Scan. The lump also tested with top marks for hormone receptors, so Tamoxifen would be very effective. She was apparently a borderline case as far as Chemo was concerned, so having discussed it with her Oncologist and Surgeon she opted not to have Chemo. So she had a 3.5 week course of Radiotherapy, which she finished about 3 weeks ago, and then started Tamoxifen a few days later.

SO...... Within 2 weeks of starting Tamoxifen, she experienced severe pelvic pain, and ultimately stopped taking it. (She has had Endometriosis in the past, so wondered if it had enflamed this). According to her Gynecologist, this is because Tamoxifen doesn't stop estrogen production at all (contrary to what we thought!). It merely blocks it from the breast area in some way. So, your body then thinks you need to produce more and sends the Ovaries into overdrive producing it!

So, the Oncologist has now brought up the question of Ovary removal. - I have to say, the Oncologist seems to be quite gung-ho in wanting to do anything to attack the Cancer, and not particularly focusing on the side effects of any course of action. The Gynecologist is a bit more cautious though and is wanting to investigate whether it's really the best long term solution.

The other question is whether with persistance, the painful side effects of Tamoxifen would pass - if so, how soon (weeks/months if ever....) or whether Ovary removal is the better option.

She is also concerned about the prospect of weight gain following Ovary removal - so any experience of this or any other related issues please feel free to comment! I've also read some pretty scary statistics about Ovary removal in younger women (particularly below 38 / to an extent below 46) can increase chances of Dementia & cognitive function. Anyone have more info on this?

    I'm really relieved and glad to read that the side effects haven't been too dreadful for you. Phew! I'm amazed you're still having your period too. It really is different with every single woman as so many of these posts prove. I'll bear in mind your advice regarding the brand, seems logical to stick to one make doesn't it? From family experience we know not all Tamoxifen are equal. My grandmother swore the earlier formula for it back in the early 80's was friendlier maybe even milder (as she was on it for 10 years post bc) when she first had it, than the later dose she had to take in the last 5 years. She deemed it fierce! 

    For me it feels like the tricky bit will be sorting the wheat from the chaff. At the minute I worry I'm getting menopausal symptoms and suffer from appalling headaches and fatigue, when I'm not that old or truly going through it yet. This why I'm worrying about the Tamoxifen compounding things. Knowing what's causing what might be pretty difficult. 
    Obviously it's early days for me still and I'll be seeing my consultant  in two weeks time for the biopsy results. It will be good to know the bigger picture and their plan in more detail. The pro op sketch was lumpectomy with LICAP partial reconstruction, followed by radiotherapy then Tamoxifen. However the biopsy results may change things of course.

  • Hello, I realise you posted this years ago but it sounds so similar to my case, can you tell me how things turned out? Did your girlfriend have her ovaries removed ? I'm waiting for a gynae app. to discuss my options, in the mean time I'm off the tamoxifen & back to my old self again I really can't believe what a difference it's made to my quality of life.

    I saw your post asking the boyfriend and I'm wondering too! I've been taking tamoxifen for about 3 weeks and from day 4 have had the worst stomach pain/pelvic cramps. They are low down on the left and at first my GP thought it was diverticulitis and have been on lots of antibiotics. I then thought it could be the tamoxifen! So have been told to stop it for 2 weeks to see if my pain resolves. I can hardly stand up straight at times with the pain so I hope it does too!

    I'm back at work & able to function like a proper human being again, however, I have a gynae app on Tuesday so I'm sure it will all change there once I have to make another difficult decision, so in the meantime i'm enjoying feeling well.

    I hope you feel as well as I do off the tamoxifen as it took me much longer to realise that it was the drug that was doing so much damage, I wish you well on your journey, all the best.

    So glad you're doing well and sound so positive. i'm booked in for surgery on 18/8 and have been told I'll be on a hormonal treatment drug probably Tamoxifen.  
     

    what manufacturer did you find had the least effect on you. Appreciate that we will all react differently to the drug. I'm just worried about the fatigue side effects.

    At the moment I am taking Tillomed brand which seems to suit me fine although my pharmacy tend to give me a different brand every time despite requesting the same brand. If you read about the different brands online you will see that they do effect everyone differently so I guess just see how you get on with them. I take Biocare hair and nail complex along with Fish Oils which seems to help counteract some of the side effects.

    I have just read your post. I too had a lumpectomy with LICAP reconstruction. I am having my radiotherapy planning appointment next week. But if all of this I am least looking forward to taking tamoxifen. I am really worried about the impact of it on me.
    How have you got in with your LICAP? Did it take long for full feeling to return and the hardness to ease?
