Cancer diagnosis' aren't set in stone. They can change from core biopsy, to full removal.  For example, some people get told they have triple positive breast cancer, they go in and find some triple negative cells. Same goes for grading. There's a few stories on here from people having their grading adjusted as the doctors get more detailed biopsies.

Scans too. Each scan etc is a snapshot from that moment in time.
