Bisphosphonates for bones after chemo and dental work!

Hi there,

I have an early brest cancer, surgery, radiation and two days ago I had last TC chemo. The oncologist suggested that in two weeks time I have my first Bisphosphonates injection. This will be zoledronic acid into a vein for every 6 months. I am planning to see a dentist soon and I am sure we will need to do some dental work... (I already have 4 dental implants and one needs some additonal work). The Maccmillan broshure given to ne state that it is importnat to avoid dental tretments acceting a jow bone when having bisphosphinates. My questions is for how long? what if I one of my tooths needs to be extracted as an emergency and I am 3 months after the zoledronic acid injection. I am bit puzzled by this as I understand the zoledronic acid could create necrosis as the bone around the extracted tooth cannot heal properly due to zoledronic acid. I am sure some of you have these thoughts before so I would be grateful. My oncologist only causaly asked me about if I am planning any dental work soon... hmmmm

    I was told it's very important to get a dental check up before you start zoledronic acid.  And then you can ask your dentist all your questions as they'll have other patients on aledronic or zoledronic acid, both bisphosphonates.  I certainly wouldn't risk taking the bone infusion due to jaw necrosis until after your dental work has been done and your dentist gives the go ahead.  You then have to be really careful with your teeth for as long as you're getting the bone infusions.  I've put mine off for now but ask if they can keep you in the booking system for a time ahead as they say that's easier and you won't get forgotten about.  I would take the dental part seriously, surprised your Oncologist mnetioned it so casually. I was given a 2.8% chance of jaw necrosis using my stats.  And a nurse told me it's normal to get a reaction to the very first bone infusion as a shock to the body, typical to feel a bit flu-like for a couple of days.  Hope that helps you a bit. 

  • Yup, definitely get ALL dental work done before starting.

    If you have suspect teeth/implants/root canals that are simply not settling or play up from time to time, they may need to be removed prior to starting it. From what we've learned, you need a clean bill of dental health when it comes to teeth. I've even read about some people having to have old root canals pulled prior to starting the treatment.

    This also happens to a lesser extent when radiotherapy is given in the jaw area. A non healing socket would be a nightmare. Your mouth contains a lot of bacteria (that is normal and can't be avoided), so an open would pose issues.

  • I have to have four teeth out after my assessment I have got six more radiotherapy treatments. My oncologist did not seem very worried about it but I am very worried I have a upper denture think I will have all bottom out and another denture if I can find a dentist  before I start my zoledronic acid
