Concerned after visit to GP!

Hi All, New member a 52 year old male with quite a high degree of health anxiety.

Been anxious about head/neck/salivary gland cancers for years.  Started out with what felt like a hard lump on one side of my hard palate and was referred.  The consultants said it was nothing more than just the way I was built and don't worry but it planted a seed in my mind which has always been there about head and neck issues.

Back in about 2014 I was seeing the GP about something else and mntioned whilst I was there a small nodule a few cm down from my ear lobe and a slightly larger one just underneath the submandibular salivary glands of which both of mine on each side are quite prominent, although again not 100% sure if you should be able to feel those either!  He basically said they were just lymph nodes that had obviously risen after an infection but had never gone down on that side and not to worry about it.  They had probably been there for quite some time before that appointent and in fact have been there for as long as I can remember.  Pretty much forgotten about them until a few weeks ago.

I then had a phone conversation a few weeks ago with a GP about a little sore on the cartilage of my right ear that had been coming and going for about 6 months. I only noticed it when drying my hair and took a photo for the GP.  It was just a little red spot which would scab over, go away, and then come back a little while later and the process would repeat.  She saw the photo and said it looked like a mild skin infection and to contact her if it came back again which thankflly hasn't now for a few weeks.  At the same time I had pain in my gum, again on the right side, which my dentist said was just inflamed gum from early gum disease and that side had receded.  He gave me some stronger toothpaste and that is now ok.  Problem is because of all these right sided issues I became suddenly obsessed again about those two "lymph" nodes on that side to the extent that below my ear is quite sore because I've stupidly been prodding and probing for the last few weeks.

Had a phone consultation with the GP and she said to come in so she could put my mind at rest.  She had a look and said she "thought" they were lymph nodes and mentioned that both left and right side of my jaw were a bit prominent, of which I replied "is that a good thing?" because I can only notice the lymph nodes on the right side.  She basically asked if they were getting bigger?  I have no idea but I don't think so.  I can "feel" or I am aware of it because I've been stupidly poking away and I suspect that prodding anything for two weeks would inflame it somewhat!  She didn't seem sure at all because it's the first time that she has seen it so nothing to compare it with and whilst she said that if it has been the same for 8 years odd it "probably" is nothing but in my stupid reading of Dr Google I am paranoid about slow growing cancers either in neck or salivary glands!

She has referred me to the Urgent Head and Neck Clinic and said if I hadn't heard anything in two weeks to give her a call.  Not sure if she is just covering herself, trying to put my mind at rest or if she is genuinely concerned about something.  For a GP to not be sure if it is lymph nodes was quite concerning.  On top of that I am a Type 2 Diabetic and my last HBA1c wasn't the best so going into hospital is a concern as diabetes puts you at a higher risk of severe illness with Covid.

Worried and concerned to say the least!  Sorry for the long post.

***Update - Literally just received a phone call booking me in at my local ENT at 4.30pm tomorrow.  Not sure if such a quick appointment is a good or bad thing as I only saw the GP at 2pm today!  :shocked:

    i just wanted to say good luck for tomorrow. Let us know how it went. It sounds like a positive thing to get an appointment quickly. Either way it will put ur mind at rest so ur able to move forward. 
    also, have a little think about what questions u may have depending on diff outcomes. I find it handy to have a list ready so I feel more prepared and don't forget to ask stuff

  • Thank you.  I was thinking last night that I had an MRI scan of the head area in 2008 and 2016 for dizziness symptoms as I have since been diagnosed with vestibular hypofunction and tinnitus in the right ear.  The balance issues have been on and off since 1991 and the tinnitus since 2012.  Have also been deaf since birth in my right ear and partially in my left.  This "node" was up definitely in 2016 so I am "hoping" if anything sinister that 2016 MRI might have picked it up, although they were focusing on the brain of course.  Going to be difficult because I rely on lip reading and masks make that difficult in the current circumstances.

  • Well saw the Consultant and it is a cyst just under the skin not a lymph node.  She had a good feel all around head and neck area and assured me that there was absolutely nothing she was worried about.  She couldn't even feel what I was worried about to start with.  The "lump" I was conerned about which was lower than the "node" was just a normal part of my throat structure and she said no-one is 100% symetrical.  I asked if an ultrasound was needed and she said she could offer me one if her opinion wasn't enough to put my mind at rest but she really didn't see the need.  She had a deserved dig at me over a vice I have, I don't drink , but I have struggled over the years to completely give up smoking, but I will now try desperately to succeed with that, but she understands that with my mum on end of life care at a care home it's not the best time!  She asked me if I had any symptoms in mouth or throat but she was however reassured that I'd had an endoscopy in 2017 for constant throat clearing that was clear.  Relieved but also aware that many aren't so lucky.

    I'm delighted to hear that it was good news for you today. I am so sorry to hear about your Mum, but do try to stop the smoking as soon as you can. Doctors can only do so much. They need patients to help as well.

    sorry to hear about ur mum. It's hard to stop smoking especially when there are a lot of stresses in your life. Take your time and when you're in the right place and feel ready you'll stop. You don't need to put any more stress on yourself!

  • Thank you Jen.  I'm still not convinced but will have to try and persuade myself that they "are" the experts.  Wish I had asked if I could have had the cyst removed but I guess under the earlobe on the neck is not the easiest place and it wasn't offered to me anyway.  Thank you again for your kind words.

  • Hi all, just an update.  Wasn't over happy with the ambiguous first "diagnosis" so asked if I could be sent for an ultrasound just to put my mind at rest.  Had one done and nothing showed up and that consultant thought it was nothing more than a raised lymph node and nothing to worry about.  Easy to say but when one consultant says it's a cyst and another says it's a gland you do wonder!  Anyway I put it to bed as I had seen two experts and neither was concerned.

    My GP then changed some of my meds and I went through a period of intense sweating.  Really bad when excercising and even just standing in the supermarket I burnt up.  Decided to come off the new BP med Ramripril for a few days and thankfully the sweating cleared up.

    The GP then did some bloods to check my diabetes levels but she was a little concerned about my regular blood work which showed abnormal results in Lymphocyte and white blood cell count - both elevated, so she wanted to redo them in a month.  They redid the tests after my White Blood cells were 11.8 and my Lymphocytes were 5.3. Second round of results WBC at 12.9, Lymphocytes had dropped to 3.5 but my Neutrophils had suddenly shot up from a normal 5.3 to 8.2.  I had been on antibiotics that week for an infected insect bite so hoping that may be the cause but she chatted with me and said she would just run the results by a haemotoligist.  I sent her a document with all my "abnormal bloods" since 2002.  At times my Neutrophils, Lymphocytes, White Blood Cells and Mean Corp Haemoglobin can always go into the abnormal range but they all usually bar the Haemoglobin come back to normal, then a few months later they will go higher again.  I guess what concerns her is my last three WBC readings have been 11.2, 11.8, 12.9 all within a couple of months, although back in 2019 they went 10.7, 10.5, 13.0, 10.0, 12.0, 7.9 within three months.  Likewise Neutrophils have hit the 9's but been back in normal range a month or two later so I always assumed that this was normal for me. 

    Maybe as she is a new GP she is just being cautious but this morning I received a phone call saying the hospital will be in touch because they have received an urgent referral.  A far cry from just "running them past a haemotoligist".  Anyone know how high these readings need to be to be a concern as I'm a tad worried now?!
