Narrowing airway end of life?

I have written about my precious brother many times.

this month is 5 years since he was diagnosed with stage 2 testicular cancer but due to him being so severely mentally disabled treatment has not been possible.

 I have been told today that the coughing and breathing difficulties he's having are being caused by his airways narrowing and that this is probably the start of the end.

can anyone please tell me any more? How long could I potentially have left with this amazing man (he's 47)

    I have been following your brother's moving story for the past five years and that sadly due to his disability he has not been able to receive any treatment. I am so sorry to hear that he is suffering from coughing and breathing difficulties at the moment and that you have been informed that this may be caused by his airways narrowing and that his time might as a result be limited. 

    It's difficult for anyone to be able to predict how long someone may live and perhaps your brother's medical team will be best placed to tell you more about what these breathing difficulties mean and what to expect so don't hesitate to ask them questions and I hope they will be able to guide you further.

    We are thinking of you and your brother at the moment as it must be quite a difficult time for you both. He does seem to be an exceptional person. We're all here for you if you need to talk.

    thank you for replying, saying you have been following my brother's story and remember him means more than you can know.

    The gp has been waiting to speak to the hospice consultant about my brothers pain, he's already on an unusually high dose of Fentanyl patches (350) so another increase is unlikely because they are worried about side effects but also because apparently when the dose is too high it might even make the pain worse. 
    Because of his severe disability he cannot understand so would not allow injections etc.

    its just heartbreaking and now this new decline
