Prostate cancer - diagnosis and treatment issue

My father was diagnosed with prostate cancer  18 months ago he had hormone treatment and then radio therapy and was told he was more or less cured. All was well. Something was missed though somewhere as he had a pain in his shoulder which was seen to be cancer in his bones. The time between scans seems to have been too long and something was missed and he died after 2 doses of chemo having been told the pain he had had was scar tissue on the prostate from the radio therapy. We were told it prostate cancer it's common and he'll be fine. I can't help thinking something was missed in his treatment somewhere as by the time he passed it had spread all over his body. My post is not to lay blame but  My advice is push push push for the correct treatment. Trust your body not the process.
