Help available?

My sister is terminally ill with lung cancer. She has bravely fought if over the last two years, but now it is catching up with her. She is very fatigueed and lies on the sofa most of the day trying to sleep. Can anyone advise how we go about getting support and the kind of support we can ask for as her health worsens. Do we go through her doctor, the local hospice, McMillan or whoever. Thank you.
