Stage IV throat cancer

My husband has just had treatment for throat  cancer. The tumour is T1 but as it's spread to nearby lymph nodes the secondary cancer is N3b. This puts the staging  at stage 1V. The treatment ( radiotherapy) was intended as curative but if it's stage 4 is that possible. Is the outlook better if it's the secondary cancer that's 4 rather than the primary?
