I am very sorry to hear about your brothers situation, this is understandably a difficult time for you as a family.

Unfortunately Lepto Meningeal Disease LMD can be difficult to diagnose but often doctors use both imaging such as a MRI scans alongside spinal fluid analysis to try and diagnose this.

Sadly chordomas can spread to the leptomeninges (the protective thin layers of tissues surrounding the brain and spinal cord). You can read more what are leptomeninges metastases? on the website.

I think it is important to talk through with your brothers specialist team about your concerns about not having a confirmed LMD diagnosis and if there could be any other causes for his symptoms.
