Unsure about breast exam I received from my GP and the routine referral I've been given. Should I get a second opinion or ask for a 2 week referral instead?

I’m hoping for some advice following a recent GP appointment.

I’m a 35-year-old female currently undergoing investigations for various ongoing symptoms. I recently saw my GP to follow up on a swollen left supraclavicular lymph node that has been present for about a month.

During the appointment, I also mentioned an area of thickened, hard tissue below my left collarbone, in the upper outer chest/breast area. It measures approximately 7–8cm, feels uneven and immovable, and is painless. Occasionally, my whole breast feels tender, and the skin over the area has been quite itchy, although it looks normal. The area is more noticeable when I’m lying down.

The GP examined it briefly, compared both sides, and agreed it was noticeable. He did not examine me lying down or perform any further assessment. He said he could refer me for an ultrasound “if I want,” but wasn’t sure the referral would be accepted. He described it as likely “tense muscle due to stress,” said it would be unusual for anything sinister in that area, and advised that it would be a routine referral and not to chase it for at least four weeks.

I initially felt reassured, but now I feel uncertain about the plan.

I understand that NICE guidelines recommend a 2-week wait referral for anyone over 30 with an unexplained breast lump.

My questions are:

  1. Does what I’ve described qualify as a breast lump?
  2. Does the examination sound appropriate?
  3. Why might a referral for imaging not be accepted?
  4. Would you recommend seeking a second opinion or asking specifically for a 2WW referral?

    I am sorry to hear about the symptoms you have and the concerns they are causing you.

    If you remain worried you are best to go back and get a different GP to assess you and see what they have to say.

    As you have said there are strict guidelines that need to be met to be referred on a suspected cancer pathway. Even when GP feels a patient should be seen on this pathway the clinics themselves can reject these sometimes if they don't think the patient meets the criteria. This can understandably be as frustrating for the doctor as well as the patient.

    GPs are usually very experienced at assessing for breast lumps and changes and usually know when it is appropriate to refer someone to the breast unit. From what you have said the area you notice on your chest may not be a normal presentation of a breast lump but this is only something your doctor can comment on.     Likewise lymph nodes can also be raised for many reasons. Most commonly they are swollen due to a recent or passed infection and often these settle down on their own. It is reasonable for a doctor to wait and see if it resolves by itself before referring for an ultrasound, unless it is particularly large when a referral may be arranged urgently.

    As I say for your own peace of mind it may be worth asking for an appointment with a different doctor to get their opinion as to what may be going on.
