Cancer Aftercare - how difficult is it to transfer medical records to a different healthcare team and continue my treatments if I decide to move house?

I would be grateful for any advice regarding moving whilst having treatment.

I was diagnosed with breast cancer (grade 3 lymph node positive). Have finished chemo, surgery and radiotherapy. I have not received any scans after finishing the radiotherapy. Does anybody know is that the standard procedure? 

I am now on Goserelin, Exemestane and starting targeted therapy Ribociclib.  

I am thinking about moving in the future and wondering how simple or difficult it would be to transfer my records to a different healthcare team and continue with these treatments. 

Any advice on what the procedure would be would be appreciated.

    I am sorry to learn that you have been going through breast cancer treatment. This must not be easy.

    It's quite common not to have routine scans immediately after treatment finishes. How often you have follow-ups can depend on several things, how you are feeling, the type of treatment you have had and may still be having, such as hormone therapy, and it can vary from hospital to hospital.

    Usually, you will have yearly Mammograms. We have more information on the follow-up after breast cancer treatment on our website. Your specialist breast care nurse (CNS) will be able to tell you what your follow-up care will be.

    However, if you do experience any symptoms that are persistent and causing you concern, you can always contact your breast care team at the hospital, and they can assess you and do any necessary tests.

    If you do move, it's important to let your current breast cancer team know, as they may be able to help coordinate the transfer and ensure your records are shared with your new healthcare team.

    Once you move, it's important to register with a GP in the new area. They’ll be able to refer you to a local oncology team. As long as it's clinically appropriate, the new team will continue your current treatment. Your electronic records will be transferred via NHS systems (GP2GP), and your hospital notes can be requested by your new team.

    You can also ask for a summary of your treatment to take with you, which can help speed things up.

    Go online and register with Patients Know Best. All your GP and Hospital records will be listed together with all up coming appointments.

    I went to hospital and they notified this company which is recommended by the NHS. 

    GPs and Hospitals can access all your medical history on this service.

    Basically the NHS use this service rather than up dating their computer system which would cost a lot of money.
