Irregular Bleeding. Have been on the pill for almost 20 years. Nurse told me this was normal but it seems to be getting worse. What should I do?

I have been on the pill since I was 16 and I am now 34, been on desogesterel for at least 10 years now and do not have periods. About two years ago I started to get some random bleeding, I went to my GP to see the nurse and she just told me it’s normal and it happens and wouldn’t give me a smear test because it was within 2 years of my last one, she said even if she did it and sent off the results they would test them because I’d had a smear test so recently. I accepted this and left but I felt very annoyed that she said it was normal, because while it’s normal for some women, it’s not normal for ME. I stressed this but she kept saying it is normal. Everything you read says you should see your nurse or GP if you experience any changes that aren’t normal for you, so why was she so blasé. It made me feel like I was wasting her time and I also felt that I wasn’t being listened to. I have previously tested positive for HPV but my most recent test came back clear (I don’t remember when my last test was, it was 2021 or 2022 I think. 
The bleeding has been on and off since then, sometimes I’ll get nothing for several months then a tiny amount for a few days - last happened at the end of July for about 2-3 days, so light I didn’t really need anything more than a liner. Today (14th Aug) I’m getting very random heavier bleeding. Should I go back and demand a smear? I don’t know what to do. I suffer from pretty severe anxiety so my mind keeps flipping between ‘you’re being a hypochondriac’ and ‘this isn’t normal, go back to the Dr’

  • Id go back and see a GP rather than a nurse.

    She is correct in saying the lab will refuse the smear test if it's not due however there could be other reasons.

    Most abnormal bleeding is from hormone imbalance so could maybe perimenopause but if be asking for a referral to the gynae team if it was me.

    I can understand your concern.

    Irregular bleeding is common in young women and is not usually related to cervical cancer. If your smears are up to date then it is not likely to be related to your cervix.

    There are many causes of irregular bleeding and the NHS has good information about this.

    If you are worried about this, then have a chat again with your GP or practice nurse.
