I can appreciate you wanting to know how to proceed. There is detailed information from the NHS about seeking a care home that provides end of life care.

You would also need to have a referral to the local palliative (macmillan) care team who visit patients in care homes. All care homes are under the care of a GP surgery as well.

Care homes will state if they accept patients that may need end of life care, and also will list their staff who have undergone further training in this area.
