Why not a microdochectomy/total duct excision?

Hi,

Following the discovery of nipple discharge and a lump close by, I underwent a mammogram, scan and punch biopsy and was scheduled for a vacuum extraction of the papilloma. The consultant radiographer on the day decided that the procedure was not appropriate and it was more likely that I'd have to have a total duct excision. I met with the breast surgeon the next day and was advised that she was recommending that the excision would not be appropriate and that the nipple would be removed instead. I was totally shocked and overwhelmed. I wasn't expecting such a drastic response and didn't know what questions to ask. Now I would like to speak with the surgeon again with a clearer head and perhaps get a second opinion (as the original biopsy was clear). Has anyone else had any experience of being declined a total duct excision and what reasons were given, and how do I go about getting a second opinion. Thank you.

    It sounds like it would be reasonable for you to speak with the surgeon again to get some clarity about why they think a nipple removal is needed rather than a total duct excision that you were originally scheduled for. I am sure they have a good reason to suggest this but it would be good for them to explain this to you.

    In terms of a second opinion you would need to either ask the hospital you are under if there is a different surgeon who can review your situation or ask if you can be referred to a completely different hospital.

  • Thanks for the response, Naomi. I phoned the surgeon's office today and was told that she wouldn't be in for the next couple of weeks but I could speak with one of the nurses instead. This doesn't help me as I need to understand the surgeon's reasoning for her choice of surgery and so I've gone ahead and booked an appointment with a different breast surgeon this week to get that second opinion. Hopefully this will give me some clarity before I'm able to speak with my original surgeon again.
