Can you ask to test for abnormal cells as well as HPV at a smear test?

Hi there,

I have a really long and complicated history with smear testing and have always had abnormal results varying from CIN 1 to 3 each time ive had a test. I have had 2 x LEETZ treatments over the years and always had nearly always tested positive for HPV until my latest smear.

I have been working on a cancer project at work and just learnt that NHS smear testing no longer tests the cells of the swab if HPV is not present, and i am really worried by this. I remember the last results that i got saying i was all clear a few years ago and was moved to 3 yearly check ups (id previously been advised to go more regularly) and i assumed that this meant that my cells were not abnormal, however now i know that they were not checked.

I already have heavy bleeding and generalised pain due (never diagnosed but seems likely that its endo) but i suddenly have the worry that it could also be cervical cancer.

My next smear is not due until next year, but i am wondering if i can make a request for one sooner via my GP, and/or if that will even make a difference i.e. can i insist that they test the cells and not just for HPV?

ORRRRR, is it impossible for me to have dysplasia if i do not have an active HPV infection and therefore i am panicking over nothing?

Thanks so much!

  • Hi NadeyG, and thank you for posting.

Nearly all cervical cancers are caused by the HPV virus. The cervical screening does not test for cancer; it tests for HPV. It is HPV that causes abnormal cervical cells, which, if left untreated over time, may become cancerous.

If the result is HPV negative, then the risk of cervical cancer is very low, and there is no need to check for abnormal cells. If HPV is found, then a test is done to check for abnormal cells.

You cannot ask for an earlier appointment for the screening or to have a test to look for abnormal cells if there is no HPV. It is good news that your last smear was clear, and it means your risk of cervical cancer is low, even with your history of abnormal cells.

HPV and abnormal cells do not cause symptoms. However, if you are having symptoms that are causing you concern, as you have mentioned, then do make an appointment with your GP. They can assess you and refer you to the hospital for tests if needed.

I hope this reply is helpful. If you have any further questions, do get back to us.
