Gynae symptoms and presence of cell changes

Had a bit of a long history of gynae symptoms over the last decade or so. In short, I had pelvic pains and then a huge cyst removed from my ovary around 11 years ago. Following that I continued to have pelvic pain and in 2015 I had a positive HPV (negative for cell changes) smear. During the smear, the nurse said she could see changes and got a doctor in to check. As I was negative for cell changes, I was put on a yearly check, which surprise surprise went from nothing to widespread, severe dyskariosis on the follow up. I was called for colposcopy and a large portion of my cervix was taken off using leep. I had a follow up and my consultant said the guy who did the procedure made a 'mess' of my cervix (yep, she used that phrase) and I had quite severe scarring, which she rather uncomfortably tried to open up for me at the time. I was told I had so little of my cervix left I would require a stitch if I became pregnant and my GP would unlikely be able to visualise so would need further smears in colposcopy.

All follow up smear were negative and my symptoms did improve for a while. In 2022 I started having unusual bleeding. I have been taking the combined pill consecutively for about 20 years and never had any problems with bleeding or any issues with my scans/womb lining ect.

The bleeding would often start after sex (penetrative or non) and would be heavy spotting for many weeks on end. I went back to the GP who sent me for coloscopy. The doctor there just during colposcopy (which was in my opinion very unpleasant and unprofessional) said she saw cell changes but just did a smear and not a biopsy. I was also sent for an US. The US saw a cyst on my ovary which had grown (and has been present on every scan since 2016) but not large enough for surgery. She also recorded a 'heterogeneous endometrium, consistent with diffuse fibroid change) . My smear results were negative for HPV, so the cells weren't tested. The only communication I had after that was from the colposcopy doctor in a short letter, saying my scan was normal and smear negative so was confident no evidence of cancer and I was being discharged and back to 3 yearly smears.

As my symptoms were continuing I asked GP for advice who advised switching to progesterone only pill (which I declined as it often causes MORE bleeding that the combined) and also advised a 4-7 pill break when breakthrough bleeding occurred. I have been following that but my periods during this time are horrific. They are lasting a month and I'm soaking through pads hourly and having unpleasant stabbing pains in my cervix whilst passing clots ect.

 I'm really beginning to feel frustrated and down with this constant cycle of bleeding and pain and don't feel I've been given any reassurance. The letter even stated that I had 'viral cell changes' seen on colposcopy but nothing was done about them and I haven't had any explanation of my hetergeneous womb lining because it was never explained by the sonographer, consultant and my GP didn't understand. 

Should I be going back or accept the symptoms as normal?

    I am sorry to hear about your situation. You do sound like you have had a very difficult time.

    From the information that you have given and as someone not involved in your care it is difficult to know how to advise you.

    I do think that as your symptoms are continuing it would be a good idea to get the advice of your GP or another GP in the practice and perhaps a referral back to the gynaecological department for their advice.

    The NHS have listed some suggestions that you may like to ask your GP. Don't be afraid to ask any questions let them know about the impact the heavy bleeding and painful periods are having on your life. Have a look at the NHS information here
