Grandfather diagnosed with stage 4 lung cancer

I am writing to you as my grandfather has stage 4 lung cancer that has metastasised to his spine and ribs. I find this very heartbreaking as I do not know how to support him and to make him feel as comfortable as he can. 

This has happened so suddenly as he did not suffer any signs and symptoms until month before being hospitalised. His signs were breathlessness and chest pain. We immediately thought cardiac arrest, however, paramedics checked his ECG and showed no changes. We took him for blood test at the  DR and they said there was some abnormality in the blood that showed his hearts was under a lot of strain but was not a concern as to health conditions/personal circumstances.

On christmas day, his respiration rate was hitting 32 per minute therefore went in by ambulance. Further tests were made with a CT scan and thats when they discovered he had a mass that was covered with infection and fluids. After a week they discovered it was stage 4 primary lung cancer and secondary bone. He currently has high calcium in his bloods and we as a family are trying to get him home where he can spend his last time rekindling with my grandmother.

My grandmother also suffers from dementia where me and my mother are currently their carers until we can figure out a future plan. 

Is there any way you can guide us how to fast track him coming home instead of going to hospice and how can we as a family support them best to their care.

    I am sorry to learn of your grandfather's situation this must be an incredibly difficult time for you and your family.

    I wondered if you have had an opportunity to discuss this with your grandfather's team who are in charge of his care at the hospital, as they will be able to plan his discharge. It sounds like he might need some equipment put in place at home such as a hospital bed, or commode.

    It would also be worth speaking with your grandfather's GP to see what support services they can offer him from the community.

    We have information on the different types of support that can be put in place when your grandfather is discharged home.

    You can ask for him to be referred to his local community palliative care nurses who are experts at managing symptoms as well as providing emotional support.
