If you can I'd suggest taking several pictures on your phone, from different angles in good light and then sending the pics to your doctor via email or online consultation form.
You could try and take a photo on your smartphone, or ask someone to do this for you. Many GP surgeries can now accept a photo via a text or email, for the doctor to see. Otherwise you could try and phone to see if the GP can do a home visit to see you. You could speak to the Practice Manager to see what your options are.
