Unfortunately, we can't tell you what the matter might be. There are various medical reasons for a bloated stomach. The most common cause is constipation, so may be check with him that he is opening his bowels on a regular basis. Some food sensitivities, and medications can cause a bloated stomach.

I would encourage him to get this assessed by a doctor just to be sensible and get a steer on it. He could make contact with his GP or hospital team, whichever is easiest.
