I notice that it I am in certain positions that one of my nipples looks as if it is flat and then at other times in looks normal enough. Can this happen.

Yes this is normal. Nipples can react to stimulus such as temperature, positions, and physical activity.
It is only if a change remains, or other changes occur as well, then it should be checked by a doctor.
NHS Scotland have information about this which might be useful.
