You may be given your hospital version of a 'My Treatment Plan' booklet by your hospital care team.

Cancer Research UK also produce a Your Treatment Record which some hospitals are using.

Do ask your treatment team about it. Some of your information may also be available on your NHS App.

A lot of hospitals do provide treatment records now but this does vary, so do ask your team where you will be having treatment. Otherwise, you are welcome to use one of our treatment records that my colleague has linked you to.
