Unfortunately, scar tissue can become harder, and more thick and fibrous over time. This chance increases as more surgery is done in the same area of tissue.

You are doing all the right things, and using vitamin E oil/Bio oil on the scar tissue may help some. If you still have contact with your original hospital breast care nurse, you can also ask them about it. Sometimes if it is painful, then further minor surgery is done to release the scar tissue, but this can have mixed results.
