Stage 4 melanoma

My otherwise well 74 year old mum was recently diagnosed with melanoma (following removal of a mole) in the time it has taken to get the second excision histology back she was admitted to a general surgical ward with abdominal pain/nausea and swollen lymph node. Quite frankly the rate of deterioration has been terrifying and whilst we wait for the result to confirm the lesions seen in her liver on MRI are secondaries, she grows weaker and more jaundiced every day. 
she is now unlikely to be a candidate for any treatment other than palliative. Can’t believe it has happened so quickly. 
what treatments are there for nausea and vomiting?

Can I insist on oncology review?

we would like her to be well enough to come home/hospice care if possible. 

How do we go about that?

    I am very sorry to hear about how unwell your mum is at the moment and this of course must be a worrying time for all the family.

    Whilst your mum is on a surgical ward her oncology team should still be involved and overseeing her care and treatment. They should also be coming to see her on their ward rounds (if they are within the same hospital). Do ask the nurses on the ward about this and if not ask if they can contact them and arrange for them to review her in person. They may also be able to co-ordinate a time for you to be able to speak with them alongside your mum to talk things through.

    The doctors may be limited in what they can say until they have the results back from the MRI. In the meantime the team should be managing her symptoms with anti sickness medications that can be given intravenously alongside fluids. It is also important that the doctors establish exactly what is causing the sickness in the first place to best manage this. Sometimes an ng (nasogastric) tube is passed through the nose and down into the tummy can help settle sickness when and where there is a blockage, but of course if these lesions in the liver that are found to be cancer anti sickness drugs are the best way of controlling this.

    I hope you find out more soon and the sickness starts to settle. Please wait and see what the complete picture looks like once the doctors have gathered all the information they need. No one can say right now what treatment may or may not be available to your mum so sit tight and take things one day at a time for now.
