Clear ultrasound with slightly raised CA125

For the past 12 months I’ve been having odd periods and some very light bleeding.  Smear in Jan was fine, spoke to GP and she did some tests, all fine except vitamin d low and slightly anemic, but I always am and I take iron supplements.  GP ordered an ultrasound in Jan and it was clear. Had a change in medication about nine months ago which has caused diarrhoea so I had an appointment with a nurse practitioner.  Nurse did some more bloods for inflammation and a few stool samples.  Everything fine except CA125, which she didn’t explain when I asked what tests she had ordered.  CA is raised but not massively at 48.  The nurse has now panicked and done a 2WW referral which I’ve almost cancelled as GP has made a routine gynaecology referral which I still waiting for.  My question is, all of the information, including information on this site says you only refer if CA125 high AND there’s something on a scan so why have I been referred.  Tried to speak to the nurse but I was told she wouldn’t call back and to just go to the appointment.  Also, what tests will the hospital do given I’ve already had a clear ultrasound?

    As I am sure you are aware a ca 125 can be raised for many different reasons. Any inflammation in the pelvic region including periods can cause this level to raise. A ca125 isn't a stand alone test and usually as you say is used in combination with a pelvic ultrasound and a gynaecological assessment to establish the cause and possible risk of a more serious cause.

    I can't say what they are likely to do when you are seen at your appointment however they may wish to repeat the ultrasound or take a look at the recent ultrasound report, examine you and ask you some questions about the symptoms you are having.

    Whilst you say are already awaiting a routine gynae appointment, this often involves a long wait. You may find that being assessed sooner could put your mind at ease and give some more information that is useful to what may be going on.
