U5 ultra scan Thyroid

Had a scan 6 weeks ago due to lump on neck. I had a 4cm U2/3 thyroid lump. Had FNA came back negative. Had a NHS scan 2 weeks ago. Very different report. I have a 5.9cm lump suspicious U5. How can two scans be so different also what is the likelihood that it is cancerous if given U5?

    I am sorry that you feel confused about recent investigations you have had resulting in different reports. I cannot say exactly what is going on as I am not part of your hospital team. I would encourage you to discuss these results with the medical team you are under. 

    The U (Ultrasound) classification is the likelihood of a nodule being malignant. This is the impression that the radiographer gets from what they can see on the screen. This likelihood of malignancy (cancer) rating is therefore based on their suspicion that this is cancer and is determined by different factors like size, shape, and what tissues the nodule is made up of. A U5 rating is the highest likelihood or suspicion of being malignant at between 70-90%. 

    As with all suspicious lumps that are being investigated a visual examination, an appropriate scan, and a biopsy or fine needle aspirate (FNA) is needed to give a full and accurate picture. Whether a thyroid nodule is rated U3, 4, or 5 on ultrasound, it is recommended that a sample of cells is needed so they can be looked at under a microscope. It is only then that a true picture of what the nodule is can be determined and a diagnosis of benign or malignant can be given.

    The size of the thyroid nodule isn't necessarily what is important, it is more about the cells within the lump.

    Do talk this all through further with your team, and let them know about your previous scans and FNA results. But they will want to follow NHS procedure and protocol.
